• Patient's date of birth.

• Landline home phone and cell phone.

• Patient email.

• Family home address.

• Name, address and telephone number of the company where you work, as well as the function you perform.

• Autograph signature of the patient.

• Data mentioned above of the father and/or mother of the patient who is a minor.

It is important to mention that you have the right to access, rectify and cancel your personal data, to oppose their treatment or to revoke the consent that you have given us for that purpose. For this, it is necessary that you send the request in the terms established by law in its article 29, to the person in charge of personal data protection, located at Calle Aldama No.550 Col. On the other hand, we inform you that your data may be transferred to entities of the same Dermydest group, with the general objective of fulfilling the purposes for which you have provided your data.
